Shortage of Anaesthetic "Green Forms"

AIPA has developed an Anaesthetic Consent Form which has been in general use in the hospitals for many years and is simply known as "the Green Form". This form is available for download by members of AIPA free of charge. However, you do need to be a paid-up member of AIPA to avail yourself of this facility.

There is currently a shortage of these forms in some areas as AIPA is in the process of updating the form in light of the Consumer Protection Act and as soon as the printers have received the changes, the new run will commence and it will be availble on the website as well as in the hospitals.
